Porges<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Voici une <strong>synth\u00e8se en fran\u00e7ais <\/strong>de l\u2019article de <a href=\"https:\/\/www.frontiersin.org\/journals\/behavioral-neuroscience\/articles\/10.3389\/fnbeh.2025.1659083\/full\"><strong>Stephen W. Porges (2025), \u201cPolyvagal Theory: A Journey from Physiological Observation to Neural Innervation and Clinical Insight\u201d<\/strong>, publi\u00e9 dans <em>Frontiers in Behavioral Neuroscience<\/em><\/a><\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>1. Introduction \u2013 Une th\u00e9orie n\u00e9e d\u2019un pont entre la physiologie et la psychologie<\/strong><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Stephen Porges retrace dans cet article la gen\u00e8se, les \u00e9volutions et les implications cliniques de sa <strong>Th\u00e9orie PolyVagale (TPV)<\/strong>, apparue en 1995. Son ambition initiale : relier les observations physiologiques du Syst\u00e8me Nerveux Autonome (SNA) aux comportements et \u00e9motions humaines. Il s\u2019agissait d\u2019un effort pour d\u00e9passer les cloisonnements disciplinaires entre neurosciences, psychologie, m\u00e9decine et biologie \u00e9volutionnaire.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">La TPV repose sur un principe fondamental : <strong>le nerf vague n\u2019est pas une entit\u00e9 unique et homog\u00e8ne<\/strong>, mais un syst\u00e8me \u00e0 <strong>hi\u00e9rarchie \u00e9volutive<\/strong> compos\u00e9 de plusieurs circuits qui fa\u00e7onnent notre capacit\u00e9 \u00e0 r\u00e9guler nos \u00e9tats internes et \u00e0 interagir avec autrui. Ces circuits \u2014 <strong>ventral vagal, sympathique et dorsal vagal<\/strong> \u2014 se sont organis\u00e9s selon une logique phylog\u00e9n\u00e9tique, des plus anciens (r\u00e9ponses de survie primitive) aux plus r\u00e9cents (r\u00e9gulation sociale et \u00e9motionnelle).<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Porges rappelle que la TPV n\u2019est pas un mod\u00e8le fig\u00e9, mais <strong>un cadre int\u00e9gratif et \u00e9volutif<\/strong>, con\u00e7u pour accueillir les d\u00e9couvertes issues de la neuroanatomie, de la g\u00e9n\u00e9tique ou de la clinique. Il revendique la m\u00e9thode scientifique dite de <em>Strong Inference<\/em> : tester plusieurs hypoth\u00e8ses concurrentes, r\u00e9viser, raffiner \u2014 un processus de maturation scientifique en dialogue constant avec les critiques.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>2. Les origines de la TPV : du hasard exp\u00e9rimental \u00e0 la r\u00e9volution conceptuelle<\/strong><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">La th\u00e9orie est n\u00e9e d\u2019une <strong>observation fortuite<\/strong> pendant les recherches de Porges dans les ann\u00e9es 1960 : la <strong>variabilit\u00e9 du rythme cardiaque (VRC)<\/strong> diminuait pendant une t\u00e2che de concentration, puis augmentait \u00e0 nouveau au repos. Cette oscillation rythmique, plus tard identifi\u00e9e comme l&rsquo;<strong>arythmie sinusale respiratoire (ASR)<\/strong>, l\u2019amena \u00e0 soup\u00e7onner un lien entre <strong>attention, r\u00e9gulation vagale et flexibilit\u00e9 physiologique<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u00c0 l\u2019\u00e9poque, la recherche en psychophysiologie reposait sur le paradigme du <strong>stimulus-r\u00e9ponse<\/strong> et minimisait les diff\u00e9rences individuelles. La variabilit\u00e9 cardiaque \u00e9tait consid\u00e9r\u00e9e comme du \u201cbruit statistique\u201d. Porges a renvers\u00e9 cette id\u00e9e : la <strong>variabilit\u00e9 est un signal<\/strong>, reflet de la capacit\u00e9 adaptative du syst\u00e8me nerveux.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Son intuition a conduit \u00e0 une relecture compl\u00e8te de la relation entre <strong>cerveau, c\u0153ur et comportement<\/strong>. Loin d\u2019un simple parall\u00e9lisme psychophysiologique, il a propos\u00e9 une approche hi\u00e9rarchique : les structures les plus anciennes du tronc c\u00e9r\u00e9bral soutiennent la survie ; les plus r\u00e9centes permettent la communication, la socialit\u00e9 et la r\u00e9gulation fine des \u00e9motions.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>3. Du parall\u00e9lisme au mod\u00e8le hi\u00e9rarchique<\/strong><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Les mod\u00e8les classiques supposaient un <strong>lien direct et lin\u00e9aire<\/strong> entre \u00e9tats psychologiques et r\u00e9ponses physiologiques. Cette perspective \u201cparall\u00e9liste\u201d ignorait la <strong>complexit\u00e9 hi\u00e9rarchique du syst\u00e8me nerveux<\/strong>, o\u00f9 les circuits inf\u00e9rieurs (tronc c\u00e9r\u00e9bral, nerf vague) conditionnent le fonctionnement des structures sup\u00e9rieures (cortex, cognition, langage).<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">La TPV rompt avec cette vision en affirmant que <strong>l\u2019autor\u00e9gulation physiologique pr\u00e9c\u00e8de et conditionne les capacit\u00e9s cognitives et relationnelles<\/strong>. En d\u2019autres termes, <strong>sans s\u00e9curit\u00e9 neurobiologique, il n\u2019y a ni apprentissage ni relation sociale harmonieuse<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Cette hi\u00e9rarchie, inspir\u00e9e de l\u2019\u00e9volution, s\u2019exprime en trois syst\u00e8mes :<\/p>\n\n\n\n<ol class=\"wp-block-list\">\n<li><strong>Complexe vagal ventral (CVV)<\/strong> : sp\u00e9cifique aux mammif\u00e8res, issu du <em>nucleus ambiguus<\/em>, compos\u00e9 de fibres my\u00e9linis\u00e9es. Il permet la r\u00e9gulation sociale, la communication, la confiance et la d\u00e9tente.<\/li>\n\n\n\n<li><strong>Syst\u00e8me sympathique<\/strong> : responsable de la mobilisation face aux d\u00e9fis et dangers.<\/li>\n\n\n\n<li><strong>Complexe vagal dorsal (CVD)<\/strong> : plus ancien, issu du <em>nucleus dorsalis motorii<\/em>, non my\u00e9linis\u00e9. Il g\u00e8re la conservation d\u2019\u00e9nergie, la digestion, et dans les cas extr\u00eames, l\u2019immobilisation d\u00e9fensive (fuite, figement, dissociation).<\/li>\n<\/ol>\n\n\n\n<p class=\"wp-block-paragraph\">Ainsi, <strong>la r\u00e9gulation autonome suit une logique phylog\u00e9n\u00e9tique<\/strong> : les circuits les plus r\u00e9cents inhibent les plus anciens, permettant des r\u00e9ponses adapt\u00e9es au contexte. Sous menace, le syst\u00e8me \u201cdescend\u201d la hi\u00e9rarchie : de la socialit\u00e9 au combat\/fuite, puis \u00e0 l\u2019effondrement.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>4. Le r\u00f4le fondateur de la variabilit\u00e9 du rythme cardiaque (VRC) et de la <strong>ASR<\/strong><\/strong><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">La <strong>VRC<\/strong> et la <strong><strong>ASR<\/strong><\/strong> (rythme cardiaque li\u00e9 \u00e0 la respiration) constituent les marqueurs exp\u00e9rimentaux cl\u00e9s qui ont men\u00e9 \u00e0 la formulation de la TPV.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Porges montre que la <strong><strong>ASR<\/strong> est un indice non invasif du tonus vagal<\/strong>, donc de la capacit\u00e9 de r\u00e9gulation du SNA. Plus la ASR est \u00e9lev\u00e9e, plus le syst\u00e8me vagal est flexible ; plus elle est basse, plus l\u2019organisme est rigide ou stress\u00e9. Cette observation s\u2019est av\u00e9r\u00e9e valide de la naissance \u00e0 la vieillesse : la ASR est un <strong>biomarqueur de r\u00e9silience<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Ses premi\u00e8res \u00e9tudes sur les <strong>nouveau-n\u00e9s<\/strong> ont montr\u00e9 que :<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>les b\u00e9b\u00e9s \u00e0 haute ASR r\u00e9agissent mieux aux stimulations et r\u00e9cup\u00e8rent plus vite ;<\/li>\n\n\n\n<li>la ASR pr\u00e9dit la viabilit\u00e9 et la capacit\u00e9 d\u2019adaptation ;<\/li>\n\n\n\n<li>les pr\u00e9matur\u00e9s pr\u00e9sentent des ASR faibles, qui peuvent \u00eatre am\u00e9lior\u00e9es par le contact et la co-r\u00e9gulation.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Ainsi, la <strong>ASR est devenue une fen\u00eatre sur la sant\u00e9 du syst\u00e8me nerveux autonome<\/strong>, bien au-del\u00e0 de la simple mesure cardiaque.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>5. Les limites du paradigme de l\u2019arousal et des anciens mod\u00e8les<\/strong><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Avant la TPV, les mod\u00e8les d\u2019\u201carousal\u201d (excitation physiologique) dominaient. Ils supposaient une seule dimension d\u2019activation du syst\u00e8me nerveux \u2014 du calme au stress intense \u2014 avec une performance optimale \u00e0 niveau moyen (loi de Yerkes-Dodson).<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Porges critique cette simplification : <strong>l\u2019excitation physiologique n\u2019est pas une ligne droite<\/strong>, mais une architecture hi\u00e9rarchique de <strong>syst\u00e8mes autonomes interactifs<\/strong>. Les anciens mod\u00e8les ignoraient :<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>la contribution sp\u00e9cifique du nerf vague,<\/li>\n\n\n\n<li>les interactions entre branches sympathiques et parasympathiques,<\/li>\n\n\n\n<li>la dynamique bidirectionnelle entre cerveau et visc\u00e8res,<\/li>\n\n\n\n<li>le r\u00f4le de l\u2019\u00e9volution dans l\u2019organisation du SNA.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">En int\u00e9grant ces \u00e9l\u00e9ments, la TPV red\u00e9finit la r\u00e9gulation autonome : ce n\u2019est pas seulement la gestion du \u201cstress\u201d, mais <strong>la capacit\u00e9 \u00e0 passer fluidement entre \u00e9tats selon la perception de s\u00e9curit\u00e9 ou de menace<\/strong>.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>6. Du signal cardiaque \u00e0 la carte neuronale : naissance du mod\u00e8le polyvagal<\/strong><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Les avanc\u00e9es techniques (notamment la m\u00e9thode <strong>Porges\u2013Bohrer<\/strong>) ont permis d\u2019extraire avec pr\u00e9cision le signal ASR des donn\u00e9es de VRC, r\u00e9v\u00e9lant son <strong>origine dans le noyau ambigu<\/strong>. Cette d\u00e9couverte a transform\u00e9 l&rsquo;ASR d\u2019un simple \u201cindice de variabilit\u00e9\u201d en <strong>signature fonctionnelle du nerf vague my\u00e9linis\u00e9<\/strong>, c\u0153ur du <strong>syst\u00e8me d\u2019engagement social<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Le mod\u00e8le TPV s\u2019est alors consolid\u00e9 :<\/p>\n\n\n\n<figure class=\"wp-block-table\"><table class=\"has-fixed-layout\"><thead><tr><th><strong>Syst\u00e8me<\/strong><\/th><th><strong>Origine<\/strong><\/th><th><strong>Fonction<\/strong><\/th><th><strong>Comportement associ\u00e9<\/strong><\/th><\/tr><\/thead><tbody><tr><td><strong>Ventral vagal (CVV)<\/strong><\/td><td><em>Nucleus ambiguus<\/em>, my\u00e9linis\u00e9<\/td><td>Co-r\u00e9gulation, lien social, apaisement<\/td><td>Regard, voix prosodique, empathie<\/td><\/tr><tr><td><strong>Sympathique<\/strong><\/td><td>Cha\u00eene thoracique<\/td><td>Mobilisation, action, fuite\/lutte<\/td><td>\u00c9nergie, vigilance<\/td><\/tr><tr><td><strong>Dorsal vagal (CVD)<\/strong><\/td><td><em>DMNX<\/em>, non my\u00e9linis\u00e9<\/td><td>Conservation ou effondrement<\/td><td>Immobilisation calme ou effondrement, dissociation<\/td><\/tr><\/tbody><\/table><\/figure>\n\n\n\n<p class=\"wp-block-paragraph\">La TPV distingue notamment deux types d\u2019immobilisation :<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Sans peur<\/strong> : relaxation profonde (sommeil, allaitement, intimit\u00e9).<\/li>\n\n\n\n<li><strong>Avec peur<\/strong> : collapsus, dissociation, sid\u00e9ration.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Cette distinction est capitale pour la clinique des traumas : <strong>toutes les immobilisations ne sont pas pathologiques<\/strong> ; certaines rel\u00e8vent de la s\u00e9curit\u00e9, d\u2019autres de la survie.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>7. Les bases neurales du rythme : l\u2019oscillateur cardio-pulmonaire<\/strong><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Les travaux de <strong>Richter et Spyer (1990)<\/strong> ont montr\u00e9 qu\u2019un <strong>oscillateur cardio-pulmonaire commun<\/strong> au tronc c\u00e9r\u00e9bral synchronise la respiration, la phonation et le rythme cardiaque. Cette synchronisation explique pourquoi la respiration lente et prolong\u00e9e (notamment \u00e0 l\u2019expiration) augmente la RSA et favorise la r\u00e9gulation vagale.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Autrement dit, l&rsquo;<strong>ASR n\u2019est pas un effet m\u00e9canique de la respiration<\/strong>, mais un <strong>signal neuronal central<\/strong> \u2014 preuve que respiration, c\u0153ur et communication vocale partagent un m\u00eame r\u00e9seau d\u2019origine vagale. Ce constat \u00e9claire les effets des pratiques respiratoires, de la m\u00e9ditation ou du chant sur la r\u00e9gulation \u00e9motionnelle.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>8. De la naissance au vieillissement : la trajectoire du syst\u00e8me vagal<\/strong><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Porges d\u00e9taille l\u2019\u00e9volution de la r\u00e9gulation vagale \u00e0 travers la vie :<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Chez le nouveau-n\u00e9<\/strong> : l&rsquo;ASR sert d\u2019indicateur de maturit\u00e9 neurologique. Une ASR \u00e9lev\u00e9e pr\u00e9dit de meilleures capacit\u00e9s d\u2019adaptation et de co-r\u00e9gulation.<\/li>\n\n\n\n<li><strong>\u00c0 l\u2019\u00e2ge adulte<\/strong> : l&rsquo;ASR refl\u00e8te la flexibilit\u00e9 \u00e9motionnelle et sociale.<\/li>\n\n\n\n<li><strong>Avec l\u2019\u00e2ge<\/strong> : ASR et variabilit\u00e9 cardiaque diminuent, signe d\u2019un d\u00e9clin de la plasticit\u00e9 autonome et du contr\u00f4le baror\u00e9flexe (r\u00e9gulation de la tension art\u00e9rielle).<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Ainsi, la <strong>perte de variabilit\u00e9 autonome<\/strong> devient un marqueur biologique du vieillissement, du stress chronique et des troubles psychiques (anxi\u00e9t\u00e9, d\u00e9pression, burnout).<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>9. Neurosception : d\u00e9tecter la s\u00e9curit\u00e9 sans conscience<\/strong><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">L\u2019une des contributions les plus influentes de la TPV est le concept de <strong>neurosception<\/strong> : le processus inconscient par lequel le syst\u00e8me nerveux \u00e9value en permanence l\u2019environnement comme <strong>s\u00fbr, dangereux ou mena\u00e7ant pour la vie<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Cette \u00e9valuation ne passe pas par le cortex rationnel, mais par des circuits sous-corticaux (tronc c\u00e9r\u00e9bral, amygdale, insula). La neurosception module directement la hi\u00e9rarchie autonome :<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>s\u00e9curit\u00e9 \u2192 activation du syst\u00e8me ventral vagal \u2192 ouverture, communication ;<\/li>\n\n\n\n<li>danger \u2192 activation sympathique \u2192 mobilisation ;<\/li>\n\n\n\n<li>menace extr\u00eame \u2192 domination du dorsal vagal \u2192 figement, dissociation.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Ainsi, <strong>le comportement social d\u00e9coule d\u2019abord d\u2019une lecture neurophysiologique du monde<\/strong>, non d\u2019un choix conscient. Les interventions th\u00e9rapeutiques doivent donc restaurer les conditions de s\u00e9curit\u00e9 neurobiologique avant toute approche cognitive.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>10. Implications cliniques et th\u00e9rapeutiques<\/strong><\/h2>\n\n\n\n<h3 class=\"wp-block-heading\">10.1 Un cadre explicatif pour le trauma et la sant\u00e9 mentale<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Porges montre comment la TPV \u00e9claire les troubles post-traumatiques, anxieux, d\u00e9pressifs ou psychosomatiques. Ces pathologies correspondent \u00e0 <strong>des pi\u00e8ges autonomes<\/strong> : le syst\u00e8me reste bloqu\u00e9 dans un mode de d\u00e9fense, incapable de remonter vers la s\u00e9curit\u00e9.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Les sympt\u00f4mes \u2014 hypervigilance, isolement, \u00e9puisement, somatisation \u2014 traduisent des tentatives de survie physiologiques, pas des \u201cdysfonctionnements psychologiques\u201d. Le traitement consiste \u00e0 <strong>restaurer la flexibilit\u00e9 vagale<\/strong> et la capacit\u00e9 \u00e0 naviguer entre \u00e9tats selon le contexte.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">10.2 Applications th\u00e9rapeutiques<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Porges souligne les b\u00e9n\u00e9fices d\u2019interventions bas\u00e9es sur la <strong>co-r\u00e9gulation<\/strong> et la <strong>stimulation vagale<\/strong> :<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>th\u00e9rapies corporelles, respiration consciente, mouvements lents ;<\/li>\n\n\n\n<li>musique et prosodie vocale ;<\/li>\n\n\n\n<li>stimulation auriculaire du nerf vague ;<\/li>\n\n\n\n<li>relation th\u00e9rapeutique s\u00e9curisante ;<\/li>\n\n\n\n<li>interventions de groupe favorisant la connexion sociale.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">L\u2019objectif n\u2019est pas de supprimer le stress, mais de <strong>r\u00e9tablir la capacit\u00e9 du syst\u00e8me \u00e0 revenir \u00e0 la s\u00e9curit\u00e9<\/strong>.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>11.
